The 49ers already have enough to worry about heading into Week 3, given that San Francisco’s defense ranked last in the league in sacks a year ago, while the roster keeps working its way back from a brutal run of injuries.

Sunday’s opponent, the Arizona Cardinals, only adds to the tension this time around.

That’s because Nick Bosa tore his ACL against that exact same Cardinals team last September, ending his 2025 season early.

He spent the offseason rehabbing and made it back for Week 1, when the 49ers confirmed he’d play in Australia against the Rams while carefully managing his snap count.

Bosa got through those first two games without a setback, and San Francisco appeared to have its star pass rusher back on track heading into the meat of the schedule.

That changed Thursday, when Bosa sat out practice and appeared on the injury report with a knee and calf designation, shifting the mood around the team almost overnight.

NBC Sports Bay Area’s Jennifer Lee Chan didn’t hold back when discussing the injury on The Morning Roast With Spadoni and Shasky. Chan called it a serious situation for the defensive end and didn’t sugarcoat what might come next.

“I’m very pessimistic about what this outcome could be. This is a serious injury for Nick Bosa, and if he’s not out for the season, he’s out for multiple weeks.”

Chan also pointed to the timing of the report as a red flag, since the 49ers released it at 3:51 p.m., well after practice had already ended, and paired the existing knee note with a brand-new calf issue.

That combination suggested the team may have found something more serious than a routine tweak.

Bosa has now torn his ACL twice in his career, first in 2020 and again last season, and he remains a five-time Pro Bowler who anchors a pass rush that already struggled without him for most of 2025.

Losing him again, even for a few weeks, would strip the 49ers of their best answer for opposing quarterbacks at the worst possible time.

The timing has fueled plenty of local chatter beyond the official report, with the Bay Area sports radio show planning to spend Friday morning digging into what the injury means for San Francisco’s season.

Bosa has stayed in headlines for reasons well beyond football lately, too. Just recently, a fabricated quote falsely attributed to him went viral after a 49ers win over the Rams, forcing another round of clarification around his name.

No official diagnosis or timeline has come from the team yet, but with San Francisco facing Arizona on Sunday, Bosa’s practice participation over the next two days should offer the clearest signal of how long he might be sidelined.
